Access all Federal forms. Printing costs are $0.20 per black and white page.

Louisiana Tax Forms

Physical tax forms are not available at the library. To request a mailed physical copy, visit the Louisiana Department of Revenue website or contact (225) 219-2113 or (225) 219-2112. You can access all state forms online. Printing is available at a cost of $0.20 per black and white page.
