Access all Federal forms. Printing costs are $0.20 per black and white page.

Louisiana Tax Forms

Physical tax forms are not available at the library. To request a printed copy by mail, visit the Louisiana Department of Revenue website or contact (225) 219-2113 or (225) 219-2112. All state forms are available online. Printing is offered at a cost of $0.20 per black and white page.
